PRELIMINARY RESULTS OF AN ORIGINAL SURGICAL METHOD FOR RECONSTRUCTION OF MASSIVE ROTATOR CUFF TEARS WITH SIGNIFICANT RETRACTION AND FATTY MUSCLE DEGENERATION

- Rotator cuff tears are considered a very common problem. According to various data, up to 40% of all rotator cuff tears of the shoulder are massive. Severe retraction of tendon and severe fatty infiltration of muscle tissue in massive tears limit the possibility of anatomical reinsertion of the rotator cuff. Currently, there are many options for surgical treatment of massive rotator cuff tears of the shoulder. Objective – to develop a new method of surgical treatment for massive rotator cuff tears. Materials and Methods. We have developed and tested a new method for the surgical treatment of massive rotator cuff tears of the shoulder with severe retraction and fatty infiltration of muscle tissue. Results and Discussion. We have operated on 17 patients with massive irreparable rotator cuff tears of the shoulder, according to the proposed method. No postoperative complications were detected. The results were evaluated using the ASES, UCLA, and DASH scales. The results of surgery in all patients were assessed as good and satisfactory. All patients returned to full-fledged household activities. Conclusions. The developed method of surgical treatment of rotator cuff tears with severe retraction and fatty infiltration of muscle tissue allows for the restoration of the vertical balance in the shoulder and the restoration of the ratio of the centers of rotation of the humeral head and the glenoid cavity and get good and excellent results in most cases.
