PECULIARITIES OF SPECIALIZED MEDICAL AID ADMINISTRATION FOR PATIENTS WITH UROLITHIASIS IN OUT-PATIENT DEPARTMENT

- Objective - to evaluate personnel support and medical aid quality in out-patient departments for patients with urolithiasis in the Republic of Tatarstan. Material and methods. The analysis was performed upon official statistic data in 2009–2020.The dynamics of full-time occupied positions of urologists and nephrologists who rendered medical aid to adult population in out-patient departments was estimated. The need for positions in profiles «Urology» and «Nephrology» was calculated. Selective study of 112 case histories was performed in order to evaluate medical aid quality in out-patients departments by criteria which were presented in the order of the Ministry of Health Care of the Russian Federation № 203н of May 10, 2017 and in the clinical recommendations. Results and discussion. Nowadays there is personnel shortage in urological-nephrological services so as the tendency to the reduction of staffing with qualified personnel (decrease of the number of specialists with qualification categories in 2009-2020 in specialty «Urology» – from 43,0 to 34,2 %; in specialty «Nephrology» – from 40 to 26,7 % ). Some defects in the work of the medical doctors in out-patient departments were revealed, such as incomplete collection of family history, insufficient control of the intake of medical preparations contributing to the development of urolithiasis, absence of the analysis of mineral composition of urinary stones, ultrasound investigation of kidneys and urinary tract, spiral computed tomography (SCT); insufficient frequency of informing patients about methods of treatment, administration of peroral hemolysis in no more than half of the analyzed cases.
