RISK FACTORS AND PREDICTION OF INFECTIOUS COMPLICATIONS OF PRIMARY TOTAL HIP ARTHROPLASTY

- Despite the continuous development of operative orthopedics, the incidence of periprosthetic joint infection after primary total hip arthroplasty shows no tendency to decrease. Objective – to identify risk factors for the development of periprosthetic joint infection during primary hip replacement, as well as to create a mathematical method for predicting this complication. Materials and Methods. A single-center retrospective case-control study was conducted (sampling period from 12.07.1994 to 01.01.2025). The study included 23 patients with periprosthetic joint infection and 100 patients with an uncomplicated postoperative course. Gender, age, body mass index, primary diagnosis, Charlson comorbidity index in the Deyo modification, previous surgical interventions on the hip joint, type of arthroplasty and method of prosthesis fixation, intraoperative transfusions, and operation duration were analyzed. Results and Discussion. The analysis showed that male gender, the diagnosis of “nonunion of the femoral neck”, previous surgeries on the hip joint, unipolar arthroplasty, intraoperative transfusions, and an increase in operation duration significantly increased the probability of developing periprosthetic joint infection. Based on the identified risk factors, a mathematical model predicting the formation of complications in specific patients was constructed. The model had an overall accuracy of 86.2 %, sensitivity of 34.8 %, specificity of 98.0 %, positive predictive value of 80.0 %, and negative predictive value of 86.7 %. Conclusion. According to the authors, a positive prognosis from the model provides grounds for including a specific patient in a risk group and for the corresponding modification of the standard postoperative treatment protocol. Thus, in complex clinical situations where it is difficult to make a final decision, the obtained prognosis may prove useful.
