EFFECTIVENESS OF PREVENTIVE MEASURES IN SURGICAL DISEASES AGAINST A BACKGROUND OF DIABETES MELLITUS

- Objective - to study the effectiveness of ozone therapy and regional lymphostimulation in patients with acute and chronic surgical diseases of abdominal cavity organs against a background of diabetes mellitus. Materials and methods. 151 patients with diabetes mellitus II type were enrolled in the study; they undergone surgical interventions in acute or chronic diseases of abdominal cavity organs. 80 patients composed basic group, they were treated by traditional methods in order to prevent postoperative purulent complications and additionally by regional lymphostimulation so as general and local ozone therapy both while the operation and after it. In order to perform it in the course of surgical intervention surgeons fixed micro-irrigator in the zone of maximal disposition of lymphatic nodes in dependence on the disease character and through it they performed regional lymphostimulation by lymphotropic mixture (antibiotic, heparin, lydasa, taktivin, novocain). The treatment duration was amounting to 4-5 days. Besides they applied ozonized 0,9% NaCl solution by abdominal cavity irrigation while the operation and by intravenous injection during 4-5 days after the operation. 71 patients composed control group; prevention of purulent complications was performed in them by traditional scheme which included observation of asepsis and antisepsis rules and parenteral antibiotic therapy. Results. Complications were registered in control group (26,8% cases; 19 patients of 71 ones); more often by 2-3 complications including wound pyosis in 10 patients, subhepatic abscess - in 2 patients. 3 patients from control group died (4,8%). Complications were revealed in basic group (12,5%, 10 patients) namely wound pyosis - in 2 patients, subhepatic abscess - in 1 patient. Average duration of patient stay in hospital without complications from control and basic groups was amounting to 16,4 ± 1,12 and 11,8 ± 0,47 bed-days respectively. Conclusion. In patients with diabetes mellitus II type with acute and chronic diseases of abdominal cavity organs the application of general and local ozone therapy in combination with lymphogenic techniques decreased the frequency of purulent postoperative complications two times more.
