- The article presented the analysis of treatment of 10 patients with brain arteriovenous malformations (АVМs) of III-IV grades according to Spetsler - Martin scale. These patients were treated by direct embolizations of pathological vessels (both basic and auxilliary means of treatment). In 6 patients embolizations were followed by one-stage AVM removal, in 3 patients - by radiosurgical treatment. In 1 patient the embolization allowed to reach total obliteration of large АVМ with multiple sources of supply. The embolization was conducive to АVМ tissue thickening, diminished bleeding sickness and these facts facilitated its surgical removal. In 2 patients associated aneurysms of middle portion of anterior chorioidal arteries were embolized and obliterated simultaneously with AVMs of basal ganglia and thalamus. The results testified that direct embolization was the simple and effective approach for АVМ treatment. this approach may be used in patients with malformations of high surgical risk and in some cases of АVМs associated with aneurysms.
