Pharmacotherapy in elderly and very elderly patients poses significant challenges for clinicians. This patient population is typically characterized by multimorbidity, necessitating the regular use of multiple medications. Conversely, comorbid conditions often constrain the feasibility and efficacy of drug therapy. We present the management of a female patient with arterial hypertension, chronic heart failure (CHF), and type 2 diabetes mellitus (T2DM). Application of the STOPP/START criteria facilitated pharmacotherapy optimization, enabling the use of agents with proven efficacy across multiple coexisting conditions, particularly dapagliflozin.
