RISK FACTORS FOR PREGNANCY LOSS IN WOMEN WITH THREATENED PRETERM BIRTH

Abstarct
Preterm labour is considered a major obstetric syndrome. The World Health Organization notes a persistent upward trend in this indicator over the past two decades. Continued research into the risk factors (RFs) for PB is scientifically significant as their parameters can vary under the influence of numerous factors, including socio-economic conditions, climatic and geographical features, ethnicity, and genetics, as well as the dynamics of infectious disease incidence and the availability of specialized care. Objective – to define significant RF for early pregnancy termination in contemporary settings among patients with threatened preterm birth (TPB) residing in the Ivanovo region. Material and Мethods. A prospective study was performed at the obstetric hospital of the Federal State Budgetary Institution “Ivanovo Research Institute of Maternity and Childhood named after V. N. Gorodkov” of the Ministry of Health of Russia in 2023–2024. A total of 219 women with clinical signs of PTB at gestational ages from 22 to 37 weeks were examined. The patients were divided into two subgroups depending on the pregnancy outcome: 145 women who had term delivery and 74 patients with PTB. Pregnancy outcomes were analyzed for every woman involved in the study. The investigation program included the assessment of the following parameters: socioeconomic status, health status, anthropometric characteristics, and specific features of the current pregnancy and childbirth. Results and Discussion. A comparative analysis of clinical data in pregnant women with TPB revealed 15 statistically significant risk factors for preterm birth depending on pregnancy outcome, Conclusion. RF associated with the occurrence of preterm birth in women with TPB in the Ivanovo region were identified. Obstetric hospital physicians are advised to consider the following RF when forming a risk group of pregnant women with TPB: financial problems as a cause of family conflicts; the presence of placental insufficiency during the current pregnancy; the lack of awareness on the need for preconception care; average monthly income below the subsistence minimum; the fetal growth restriction during the current pregnancy; the difficulty in self-assessing one’s health; a history of vaginitis, pre-eclampsia, threatened abortion and placenta previa during the current pregnancy; failure to receive social benefits; vibration in the workplace; the lack of shared interests in the marital couple; a history of abortions; and insufficient consumption of fresh vegetables.
