“SMALL” FORMS OF ENDOMETRIOSIS: CLINICAL FINDING

Abstarct
Objective - to study the peculiarities of endometriosis clinical picture in women with infertility and “small” forms of the disease and to reveal its risk factors. Materials and methods. 350 women with infertility were examined; they have been observed under conditions of specialized consultation; external genital endometriosis was confirmed by therapeutic diagnostic laparoscopy and subsequent histological examination of focus or biopsy material of peritoneum. Basic group was formed of 264 women with I-II stages of endometriosis, comparison group was formed of 86 women with III-IY stages of endometriosis. 103 healthy fertile women who realized their reproductive function composed control group. Clinical laboratory and instrumental examination was performed; patients’ inquest was made in order to reveal possible factors of endometriosis development and concomitant abnormities. Results. Various disorders of menstrual function, varied gynecologic and concomitant abnormities were trustworthy frequently revealed in women with endometriosis in comparison with control group patients. Algomenorrhea, primary infertility, operations for ovarian cysts were rarely observed in women from basic group in comparison with patients with III-IY stages of endometriosis (comparison group). By ROC-analysis it was determined that risk for the development of “small” forms of endometriosis increased in the presence of such factors as inopportune formation of menses, allergic reactions, inflammatory diseases of small pelvis organs, operative interventions on small pelvis organs, presence of bacterial-viral infection. Conclusions: 1. “Small” forms of endometriosis in patients with infertility were characterized by minimal clinical manifestations and by the absence of significant anamnesis risk factors; it hampered early diagnosis of genital endometriosis of I-II stages and infertility diagnosis. 2. ROC-analysis demonstrated that such factors as inopportune formation of menses, allergic reactions, inflammatory diseases of small pelvis organs, operative interventions on abdominal cavity organs, presence of bacterial-viral infection increased the risk for “small” forms of endometriosis development.
