THE USE OF INTERNATIONAL CLASSIFICATION OF FUNCTIONING, DISABILITY AND HEALTH FOR THE ASSESSMENT OF REHABILITATION AND ABILITATION IN PREMATURE INFANTS

- Objective - to reveal the factors which influence the formation of general ability categories in the frames of complex rehabilitation and abilitation performing and to assess the rehabilitation efficacy by International classification of functioning, disability and health for children and adolescents. Materials and methods. 40 patients who were observed from the date of birth at Pediatric republican clinical hospital of the Ministry for public health of the Republic of Tatarstan. 29 infants got complex rehabilitation therapy within first year of life. Psychoneurological profile of development was evaluated; motor function, perceptive functions (visual and acoustic perception), speech functions (expressive and impressive), cognitive development were examined and development average index was calculated. The disorders of structures, functions, activity and participation were estimated by well-known scales and classifications. Results. The authors detected statistically significant decrease of muscle tonus after the treatment in 14 (35%) infants. The correlation analysis demonstrated the interrelation between periventricular leucomalacia degree, paresis manifestation (r = 0,52, p<0,01) and invalidism presence to the first year of life (r = 0,4, p < 0,05). There were established more high parameters in the categories for the orientation and movement after complex rehabilitation therapy. The ability for movement was decreased in large volumes of cerebral disorder, expressed neurological deficiency and somatic troubles. The most rapid beginning of rehabilitation therapy at third stage of nursing resulted in the diminishment of disorders manifestation in such category of patients. Conclusions. The effectiveness of complex rehabilitation was determined by the improvement of the abilities to movement and orientation, increase of the degree of drawing in vital situation in premature infants. The main modifiable factors which defined the mobility and ability for self care to the first year of life were proved to be the beginning terms of rehabilitation therapy at third stage of nursing, the manifestation of neurological deficiency and somatic well-being in premature infants.
